No. The Rich Forms app runs solely in your SharePoint environment or Office 365 and does not need any connection to an external site or server.
Rich Forms uses the new Office app model that was introduced in SharePoint 2013 and Office 365. It does not run on SharePoint 2010 or older.
Since Rich Forms basicly uses standard HTML changing styles can be done using CSS or inline styles. This needs some CSS knowledge and embedding style tags in the content editor though. We are developing a more user-friendly styling feature and will deliver this as a free update in the Pro version soon.
Yes, Rich Forms can be used in an Office 365 public website. We actually use it on Ardevia’s Office 365 public site for the support form. It does need an extra user solution to make a list editable for anonymous users in public websites: http://anonymous365.codeplex.com/Otherwise the list forms are only available to logged in users.Also the ribbon actions are trimmed by SharePoint so that you won’t find the “Customize Rich Forms” ribbon action on a list view. So you will need to use the main app page to start customizing a form. You can reach the main app page by going to Site Contents and then selecting the “Ardevia Rich Forms Pro” app.
